SUMMARY

Neuropsychology is defined as the study (knowledge, evaluation and modification) of the brain-behavior relationship. Neuropsychology seeks to understand how the brain, through the neural network, produces and controls the behavior and mental processes including emotions, personality, thinking, learning, memory, focus, problem solving and knowledge. This discipline tries to identify the biological reasons for the behavior. Among other purposes, it seeks to clarify how diseased or damaged brain structures alter behavior and interfere with the mental process and cognitive functions.

The neuropsychological tests try to measure the deficit in cognitive function and the behavioral capacity that can result from brain damage by trauma, attention deficit, exposure to toxic chemicals, alcohol, convulsive disturbances, Parkinson's disease, stroke, dementia, psychosis, etc.

These tests evaluate different domain areas, namely:

 • orientation • memory

• attention / concentration

• perception

• language

• praxis

• calculation

• visuoconstructive skills

• visual perception

• executive / adaptive functions

• emotional domain

 These tests do not establish a diagnosis, but quantify the severity of the alteration of certain intellectual activities. The diagnosis must always be made based on the clinical history and according to the criteria of Diagnostic Statistical Manual-IV-TR (known by its acronym in English as DSM-IV-TR).

OBJECTIVE

The objective of this policy is to determine which neuro psychological test is the appropriate in different scenarios and evaluate its clinical outcome.

POLICY STATEMENTS

Neuropsychological tests are considered for payment in the following situations if they are administered by a neuropsychologist as ordered by a neurologist:

 1. Establish a differential diagnosis between organic and functional pathology.

2. Evaluation of dementia vs. Pseudodementia

3. Differentiate epilepsy from pseudoconvulsions.

4. Differentiate sequelae from brain trauma simulation ("malingering").

POLICY GUIDELINES

Areas of domain to be evaluated by neuropsychological testing should include intellectual, spatial memory, executive functions, adaptive functions, personality, language, emotional control and attention.

According to Law 163 of August 13, 2024 - “Law for the Protection, Security, Integration, Well-being and Comprehensive Development of People with Autism Spectrum Disorders”, medical plans are obliged to offer coverage for all health interventions scientifically validated as effective for Autism Spectrum Disorders and may not establish limitations regarding age, limit of benefit, or number of visits to a medical services professional, once medical necessity has been established by a licensed physician.

BACKGROUND

Neuropsychological testing usually involves extensive evaluation of multiple cognitive domains (eg, attention, orientation, executive function, verbal memory, spatial memory, language, calculations, mental flexibility, and conceptualization).

In a 2001 practice parameter, the American Academy of Neurology (AAN) reviewed a number of studies of neuropsychological testing for dementia; some were well-designed observational controlled studies [49-53], and others were case series . Most studies demonstrated a relatively high sensitivity (range 80 to 98 percent) and specificity (from 44 to 98 percent) for detection of dementia. The AAN concluded that neuropsychological batteries are useful in identifying patients with dementia, particularly when administered to those at higher risk by virtue of memory impairment. Neuropsychological instruments that emphasize memory function were considered most useful. Five subtests (animal naming, Modified Boston Naming Test, MMSE, constructional praxis, and word list memory) were identified to be a valid, reliable measure of cognition in normal aging and AD. An aggregate total score accurately differentiated normals from those with MCI and AD. However, it is important to recognize that scores can also be influenced by education and age and apply only to individuals whose primary language is English.

In addition to its role in helping to discriminate between normal aging and dementia, neuropsychological testing can identify patterns of deficits that suggest a particular cause of dementia (eg, AD versus DLB). However, no single item on the panel has high discriminatory ability, and the pattern of performance on various domains must be interpreted in conjunction with the history, neuroimaging studies, and other testing to arrive at a presumptive etiologic diagnosis.

RATIONALE

Assessment of the diagnostic technology in relevant populations and clinical utility and demonstrate that the diagnostic information can be used to improve patient outcomes.
