Obstetric Ultrasonography is an ultrasound examination with an instrument in real time, using transabdominal or transvaginal approach or both. Real -time ultrasonography is needed to confirm fetal vitality by observing fetal cardiac activity and fetal movement. Selecting transduce frequency is a compromise between penetration and resolution of this. Several types of tests, standard or basic, limited, and specialized (or detailed level II) these types of sonograms are performed in the second and third trimester of pregnancy. The first quarter is different and is discussed separately.
The objective is to review fetal anatomical evaluation in different stages during normal and at risk pregnancies.
Ultrasonographic evaluation of the fetomaternal complex in the different stages of pregnancy is considered clinically indicated and medically necessary.
A standardized test is performed during the second and third trimester of pregnancy. It includes an assessment of fetal presentation, amniotic fluid volume, cardiac activity, the position of the placenta, fetal biometry, and an anatomic study. If technically feasible, the neck of the uterus and appendages are also discussed.
A limited examination is performed when a specific area requires investigation. In an emergency, for example, in a patient bleeding could conduct a limited examination to assess fetal heart. This assessment would also be appropriate to verify fetal presentation in a patient in labor, but in most cases, limited ultrasound examinations are appropriate only when a thorough examination has been registered.
A detailed anatomical examination is performed when a fetal abnormality is suspected based on history or abnormal biochemical findings of either limited or standard screening.
Other specialized tests include Doppler fetal biophysical profile, fetal echocardiogram or additional studies biometrics.
First Trimester Ultrasound
Indications
A sonographic examination can be beneficial in many circumstances in the first trimester of pregnancy , including but not limited to, the following:
• To confirm the presence of an intrauterine pregnancy
• To evaluate a suspected ectopic pregnancy .
• To define the cause of vaginal bleeding.
• Assess pelvic pain.
• To estimate gestational age.
• To diagnose or evaluate multiple pregnancy.
• To confirm cardiac activity.
• To complement the chorionic villus sampling, embryo transfer, and location and extraction of an intrauterine device (IUD).
• To evaluate maternal pelvic masses and / or uterine abnormalities.
• To evaluate suspected hydatidiform mole.
A limited examination may be done to evaluate interval growth, estimate the volume of amniotic fluid, assess the cervix, and assess the presence of cardiac activity.
Scanning in the first quarter can be performed either transabdominal or transvaginal. If a transabdominal examination is not definitive, one ecografiatransvaginal or transperineal should be performed whenever possible.
a. The uterus and annexes must be evaluated to determine the presence of a gestational sac. If a gestational sac is seen, its location should be documented. The rump length is more accurate indicator of gestational age
(menstrual) that the average diameter of the gestational sac. However, the average diameter should be documented gestational sac when an embryo is not identified.
Caution should be exercised in making a presumptive diagnosis of a gestational sac in the absence of a definite embryo or yolk sac. Without these findings, one intrauterine fluid collection could represent a pseudogestational sac associated with ectopic pregnancy.
b. Presence or absence of cardiac activity should be reported. With transvaginal scans, cardiac motion is usually observed when the embryo is 5 mm or more in length. If an embryo less than 5 mm without cardiac activity is seen, further screening at a later time may be necessary to support heart activity.
c. Number of fetuses should be reported. Number of amniotic and chorionic membranes must be documented for all multiple pregnancies when possible.
d. Evaluation of the uterus, adnexal structures, and cul -de- sac should be performed. The presence, location and size of leiomyomas and masses in the Annexes should be recorded. The cul -de- sac must be analyzed to determine the presence or absence of liquid.
Indications
Ultrasound can be beneficial in many situations within the second and third trimester, including, but not limited to, the following circumstances:

• Estimation of gestational age ( menstrual )
• Evaluation of fetal growth .
• Vaginal bleeding.
• Abdominal / pelvic
• Determination of fetal presentation.
• Suspected multiple pregnancies.
• Adjunct to amniocentesis.
• Significant discrepancy between the uterine size and clinical dates.
• Pelvic mass.
• Suspected hydatidiform mole.
• Attached to the cerclage.
• Suspected stillbirth.
• Suspected uterine anomaly
• Assessment of fetal wellbeing.
• Suspected amniotic fluid abnormalities.
• Adjunct to external cephalic version.
• Cases of premature rupture of membranes and / or preterm delivery.
• Abnormal biochemical marker.
• Follow-up evaluation of a fetal abnormality.
• Follow-up evaluation of placental location when there is suspicion of placenta previa.
• History of previous congenital anomaly.
• Evaluation of fetal condition when the patient enters late prenatal care
In certain clinical circumstances, a more detailed examination of fetal anatomy may be indicated.
• Fetal cardiac activity, the number of embryos or fetuses and presentation should be reported.
• In certain clinical circumstances, a detailed examination of fetal anatomy may be indicated.
• Multiple pregnancies require additional documentation:
• Detail of the amniotic membranes
• Comparison of fetal size,
• Estimate amniotic fluid volume
• Fetal genital (when displayed) .
• Placental location, appearance and relationship to internal organs should be recorded.
• The umbilical cord should be evaluated and documented number of blood vessels when possible
It is recognized that the apparent position of the placenta in early pregnancy correlates well with its location at the time of delivery. Views transabdominal , transperineal or transvaginal can be helpful to visualize the internal organs and its relationship to the placenta.
Transvaginal or transperineal ultrasound may be considered if the cervix appears shortened or if the patient complains of regular uterine contractions.
In the first trimester crown-rump measurement is the most accurate way to determine the date of pregnancy. Beyond this period, using a variety of sonographic parameters such as the length of the two-parent diameter, abdominal circumference and femoral shaft can be used to estimate gestational age.
Variability in age estimates, however, increases with advancing pregnancy. Significant discrepancies between gestational age (menstrual) and fetal measurements can indicate the possibility of abnormal fetal growth, intrauterine growth restriction, or macrosomia.
The biparietal diameter is measured at the level of the thalamus and cavum septi pellucidi . The measurement is taken from the outer edge of the skull proximal to the distal edge inside of the skull. In the measurement of the head circumference measurement is more reliable than the biparietal diameter for estimating gestational age. The circumference of the head is measured at the same level as the biparietal diameter, around the outer perimeter of the cranial vault. This measurement is not affected by the shape of the head. The length of the femoral shaft can be reliably used after 14 weeks gestation. Waist circumference should be determined in line with the skin in a cross-sectional view at the junction of the umbilical vein, portal sinus and when the fetal stomach visible.
The fetal weight can be estimated by obtaining measurements such as the biparietal diameter of the head circumference, abdominal circumference and femur length. Prediction results are diverse. Models can be compared with fetal weight percentiles published nomograms .
If previous studies have been performed, the measurement interval changes should also be evaluated for growth. Growth assessment: typically can be performed in at least 3 weeks apart. A scan with a shorter interval may lead to confusion as to whether the anatomical changes were actually caused by variations in growth as the measurement technique itself.
Today, even the best method of predicting fetal weight can cause errors as ± 15 percent. This variability may be influenced by factors such as the nature of the patient population, the number and types of anatomical parameters measured , technical factors affecting the resolution of ultrasound images , and the weight range being studied .
Evaluation of the uterus and annexes should be performed.
The fetal anatomy can be assessed by ultrasound after about 18 weeks gestational age. It is possible to document normal structures before this time, although some structures may be difficult to visualize due to fetal size, position, movement, abdominal scars, and increased wall thickness
feeding. A second or third quarter tomography can pose technical limitations for assessing anatomical image artifacts due to acoustic shadow.
The following areas of assessment are essential elements of a standard examination of fetal anatomy. A more detailed fetal anatomic examination may be necessary if an abnormality is suspected or an abnormality found on examination.

The basic cardiac examination includes a four-chamber view of the fetal heart . If technically feasible , evaluate the output of the tracts .

5. Spine : cervical, thoracic , lumbar and sacral
6. Extremities
Legs and arms - the presence or absence
7. Gender
Medically indicated only low-risk pregnancy to assess multiple gestations.
Proper documentation study is essential for high quality care to the patient. This should include a permanent record of the ultrasound images, incorporating the measurement parameters and anatomical findings. Images must be properly documented with the date of the examination, patient identification and proper orientation of the image. THE written report of ultrasound findings should be included in the patient record. Reporting should be in accordance with Practice Guide ACR .
The Society for Maternal -Fetal Medicine ( SMFM ) has stated that a fetal ultrasound with detailed anatomic examination ( CPT 76811 ) is not necessary as a routine screening for all pregnancies ( SMFM , 2004 ) . Rather, this exploration is needed when it is known or suspected fetal anatomic abnormality or genetic abnormality or fetal abnormalities increased risk (eg , AMA, diabetic fetus or teratogenic risk due to genetic problems , or abnormal prenatal assessment ) . Thus, the SMFM has stated that the practice of this study is limited to OB / perinatologist with special expertise in identifying and counseling, fetal abnormalities ( SMFM , 2004 SMFM 2012) .
SMFM has also determined that no more than a fetal ultrasound with detailed anatomic examination is necessary for pregnancy, when medically necessary ( SMFM , 2004 ,SMFM 2012 ,SMFM 2021) . Once this detailed fetal anatomic examination is done, a second should not be performed unless there are extenuating circumstances with a new diagnosis. The SMFM has stated that it is desirable to repeat the detailed anatomical examination (CPT 76811) when the patient is seen by another specialist in maternal-fetal medicine, for example, for a second
opinion on a fetal anomaly or if the patient is referred to a center tertiary special neonatal capabilities.
Considered for payment:
• A sonogram in the first quarter whose purpose will be to set the due date .
• A sonogram in the second trimester for fetal anatomic evaluation .
• If necessary one in the third quarter if there is concern about fetal growth .
• Tridimensional Ultrasound (3D) and four-dimensional fetal ultrasound (4D )

The term "ultrasound" refers to sound waves of a frequency greater than that which the human ear can appreciate, namely frequencies greater than 20,000 cycles per second or Hertz (Hz). To obtain images of the pregnant or nonpregnant pelvis, frequencies of 2 to 10 million Hertz (2 to 10 megahertz [MHz]) are typically required.
Real-time imaging is the most common sonographic technique used in obstetrics and gynecology. Multiple individual B-mode gray-scale images are obtained and rapidly displayed in succession, thereby creating a video of the area of interest over time that can be used to evaluate its structure and some aspects of its function. Real- time ultrasound is especially useful for imaging mobile subjects, such as the fetus or heart, and for quickly viewing an organ from different orientations.

Obstetrical ultrasound provides information about the pregnancy that is essential for providing optimal prenatal care: accurate determination of gestational age, fetal number, cardiac activity, placental localization, and diagnosis of major fetal anomalies. In the second trimester, fetal anomaly detection rates range from 16 to 44 percent, with detection rates up to 84 percent for lethal anomalies [21]. Ultrasound examination also improves the detection of fetal growth disturbances and abnormalities in amniotic fluid volume; however, a beneficial impact on pregnancy outcome has not been proven.
The American College of Obstetricians and Gynecologists has recommended ultrasound examination for all pregnant patients .
